Registration Documents

Please see the links below for all you need to register your child in Cookie Jar Preschool. You have the option to complete a hard copy or an electronic copy of the Child Registration form.

If using the electronic registration form, please be prepared to open the document with a different viewer. This will open up the fields for editing. Save the completed form and email a copy to cookiejarpreschool@gmail.com. Upon receipt of the $50 registration fee, which can be sent via email money transfer, your child's spot will be secured.
